Health pledge

I, the undersigned (captain name), as team captain and on behalf of its registered players, pledge that our health allows us to take part in the sport we have registered for, and that we bear full responsibility for any medical injury resulting from poor physical fitness.

We acknowledge the instructions and the protective equipment requirements; registration is not complete without this. We understand protective equipment is inspected before play.
