Health pledge

I, the undersigned (captain name), as team captain and on behalf of its registered players, pledge that our health allows us to take part in the sport we have registered for, and that we bear full responsibility for any medical injury resulting from poor physical fitness.

Declarations

The roster is up to 10 players who are citizens, children of citizen mothers or born in the country — only one of them an optional resident — plus one coach and one manager. Only names on the approved roster may play.
